Liquor License Information

Special Designated License

1. To obtain a Special Designated License (SDL) visit http://www.ci.omaha.ne.us/cityclerk/liquor.htm or Contact the City Clerk's office at 444-5324.
2. Print and complete the "SDL – Non-Profit Applicants" form.
3. Attach the "Event Diagram" to your SDL application.
4. The following is the information you need about Mutual to complete your application.

   • Question 7:
     The dimensions of the Dome are 150'x150'
   • Question 9:
     Mutual's Security Department at 351-3000
   • Question number 10 and 10a:
     Check "yes" box on these two questions
   • Question 13:
     Name and cell phone number of an on site person form the host organization.
   • Question 14:
     The person who signs as the Authorized Representative from the host organization also needs to sign the Affidavit of Non-Profit Status.
5. Hand deliver the SDL application and event diagram to the City Clerk’s office ($25 fee). You will need to apply at least two weeks prior to the event.
   A $25 check made payable to the City Clerks office and a $40 check made payable to the Liquor Control Commission are due at this time.
6. After your request is approved, the City Clerks office will send the application to the Liquor Commission in Lincoln.
7. The Liquor Commission will send the approved SDL to the City Clerks office, which will notify you when it is ready. You will have to go sign and pick up the SDL in person and post the license and receipt at the location on the event date.

The entire process takes approximately 4 weeks.
