Disease Management - Healthydirections®

The Healthydirections® program includes prevention efforts and education for asthma, diabetes, heart failure, coronary heart disease, and chronic obstructive pulmonary disease.  Healthydirections® is a voluntary, patient-centered program designed to foster collaborative effort between the participant, physician and our disease case managers to improve quality of life, optimize use of health care resources and reduce health expenses.

How the Program Works

We identify potential candidates for the program using medical and prescription drug claims information and then contact candidates and ask them to participate. The program is completely voluntary. Plan members choose whether or not to participate. Once enrolled, they receive:

  • A welcome kit and a call from the program to discuss medical history, lifestyle behaviors and current disease state
  • Follow-up calls to monitor health and support the physician's treatment plan
  • Toll-free access to a program nurse
  • Educational materials
