1. When you click “Accept and Continue” you’ll leave the Mutual Perks website operated by Mutual of Omaha Insurance Company. You’ll go to sites operated by independent vendors that have agreed to make discount offers available to Mutual customers.
2. Mutual is not providing any of your personal information to these vendors or receiving any personal information about your use of the vendors.
3. You’re free to use or not use any of the offers and can discontinue using the vendors whenever you want.
4. If you have a HIPAA-covered product with Mutual, the following also applies...I authorize Mutual to make these offers available to me and understand this Authorization is valid until revoked and will remain in effect so long as you participate in the Mutual Perks Program, which you may terminate at any time. You may revoke this Authorization at any time or obtain a copy of this Authorization by sending a written request to Attn: Privacy Office, Mutual of Omaha, 3300 Dodge Street, Omaha, NE 68175. You can choose to not accept this Marketing Authorization. Refusal will not affect Mutual product enrollment(s), your ability to obtain payment under your policy(s), or eligibility for benefits under your policy(s).
